From your very first personal telephone call answered by a professional receptionist, to coming in, taking a tour, talking to the therapy staff, meeting with the admissions team and assisting with your selection in your room- you will feel the “human touch” that is so important to us all! Crestview's Sub-Acute Rehabilitation Program offers an expanded level of service needed for Short-term Therapy care before returning to home. The flexibility of the program in caring for Residents at different stages of the therapy process makes it an ideal option for individuals beginning their developed therapy program.
Crestview's Sub-Acute Therapy Program is specialized to meet the individual needs of each client. We strive for individualized levels of independence and our goal is to meet your planned destination after recovery without unnecessary restrictions.
Following an individualized, thorough evaluation performed by our licensed therapists, a treatment plan will be customized to meet your needs and goals. Often, individuals require a combination of physical therapy, occupational therapy, speech/swallowing therapy and/or skilled nursing care as an integral link to resuming life in their community. While an individual progresses to need less assistance with specific tasks, they may require training to master activities of daily living before returning to home. Areas we focus on may include, but are not restricted to:
- Walking
- Climbing Stairs
- Bathing
- Dressing
- Transferring Techniques
- Regaining Strength and Endurance
- Organizational Skills
Crestview's Multi-Disciplinary Therapy Team is committed to restoring maximum functioning in the shortest period of time. Our Sub-Acute Therapy Program is valuable in addressing a vast scope of medical conditions such as:
- Orthopedic Surgery
- Neurological Incidents
- Cardio Thoracic Surgery
Generally, individuals in need of Short-Term Therapy are striving to achieve their goals within a relatively short period of time and are candidates for our Sub-Acute Therapy Program. Clients typically are admitted directly from an acute care hospital once their condition has stabilized.
