Les Woodgeard, R.N., Director of Quality Assurance/Sub-Acute Care — Les assures all departments are following policy & procedures, state/federal rules and guidelines. Performs studies to ensure optimal care is delivered.

Renee North, Administrator of Operations — Renee is responsible for the management of the following departments: Nursing, Infection Control, Social Services, Activities, Maintenance, Housekeeping, Laundry, Dietary, Care Conference Team, Nursing Case Management, Medical Records, Human Resources, and In-service Education. In addition to managing these departments, Renee is available for Residents and Family Members as a resource to ensure the Residents' stay at Crestview is comfortable and that all needs are being met. Renee's office is located in the Eckert Wing. .

Kelly Shouldis, R.N. Director of Nursing (D.O.N.) — Kelly ensures resident & family problems and concerns are resolved in a timely manner. Daily rounds to ensure quality care. Kelly ensures adequate staffing available; residents receive nursing care, treatments, and medications as ordered by physician.

Kim Biesk, L.P.N., Director of Human Resources/In-Service & Infection Control Director — Kim is responsible for hiring all personnel for all departments and shifts. Kim provides in-service training for all staff on educational & miscellaneous matters necessary and required for all personnel.

Susan McCormack, Admissions Coordinator — Susan coordinates all incoming residents from various hospitals discharging staff and the community at large, with supporting nursing and social services staff. Susan works with all families of incoming residents to make a smooth transition from hospital and/or home. Susan is the person to call for tours and information regarding bed availability and placement in Sheridan & Eckert Wings for rehab program openings.

Barbara Hamilton, Billing Coordinator — Barbara is responsible for monthly billing charges and statements, has an extensive on-going background in Medicare & Medicaid reimbursement.

Kim McPeek, L.S.W., Social Services Director — Kim organizes & participates in resident discharge planning; contacting appropriate agencies, visits with the resident and performs services, needs, communication with family & friends, utilizing community resources. Kim acts as a liaison between the resident, family, & staff of the facility for any concerns, which may arise. Kim works as part of the Progressive Development Meeting that consists of Social Services, Nursing & Therapy meeting with the residents and their families upon admissions to gain knowledge of the resident's prior functioning level and targeting those individual goals in the direction needed to achieve a smooth transition to discharge.

Shannon Alford, L.P.N., Case Management Nurse for Care Planning and Treatment Services — Shannon works as part of the Progressive Development Meeting that consists of Social Services, Nursing & Therapy meeting with the residents and their families upon admissions; to gain knowledge of the resident's prior functioning level and targeting those individual goals in the direction needed to achieve a smooth transition to discharge. Visits, educates, and keeps current to all therapy residents' concerns, conditions, and assists Social Services and attends all resident discharge planning meetings. Shannon is the Clinical Leader for the Falls Committee, the MDS Assessment, and Discharge Planning Team.

Theresa Nelson, L.P.N., Director of Medical Records/Case Manager Nurse — Theresa assures all departments are following policies and procedures and that medical records are in good order. Theresa is the Clinical Leader for Medical Records. Theresa is a liaison between managed care plans, insurance companies, Medicare, residents, families and physicians. As a Case Management Nurse works as part of the interdisciplinary case management team that consists of Social Services, Nursing and Therapy.

Linda Donavan, Activities Director — Linda is responsible for the direction of quality activities for the residents and company liaison for 100+ volunteers within our health care campus. Linda is in charge of planning, implementing, and organizing the monthly activity calendar considering the wants and needs of the Residents. Linda develops activities within the healthcare campus to reflect a wide range of the residents' interests while providing physical and mental stimulation. Linda is the Chairperson for C.H.A.T.S. intergenerational program.

Wilma Lea Shaffer, Director of Business Development — Wilma Lea is a liaison with the community-at-large, facilitating networking opportunities increasing the residents quality of care through non-profit fundraising efforts and events, part of the admissions, development and research teams.
