Therapy Services
Outpatient
Therapy Service is available to clients who are discharged to their homes upon successful completion of an individualized, in-house Therapy Program after a brief therapy stay. Once those Clients return home, the “potential” for falls occurs within 24 to 72 hours. This may cause the clients, once again, to be hospitalized and a possible re-admission to our Therapy Program.
In 2007, Outpatient Therapy Services became a reality offering clients a continuum of care. Crestview is dedicated to those clients that just returned home and still need that extra care once they are living at home.
Crestview Outpatient Services available:
- Aquatic Therapy
- Occupational Therapy
- Physical Therapy
- Speech Therapy
Same place, same faces, however, the clients return home to regain independence. In many cases, our outpatient services can be provided by Medicare Part B. Transportation is available to and from Outpatient Therapy appointments. Appointments can be made between the hours of 8:30 A.M. to 4:00 P.M.
- Physician referral is required.
- Medicare B insurance accepted.

Aquatic Therapy
Crestview is proud to be the only skilled facility in Fairfield County to provide aquatic therapy. Aquatic therapy reduces stress and pain associated with traditional physical therapy. Conditions that benefit from Aquatic Therapy include, but are not limited to, the following:
- Total Joint Replacement
- Cardiac Disease
- Degenerative Joint Disease
- Arthritis (osteo & rheumatoid)
- Back Strain or Pain
- Fractures
- Pain Syndromes
- Spinal Stenosis

Crestview offers over 4,400 square feet of physical, occupational and speech therapy space which is equipped with state of the art equipment, 5 treatment rooms, a separate hydro-therapy/tissue therapy room and private locker room.
Crestview maintains a close relationship with more than 90 local doctors and various hospitals to coordinate a continuum of care for its Residents. Robert J. Dominguez, M.D. is our Medical Director and is attending physician to many of the Residents.
The Arthritis Foundation uses our therapy pool four days a week, Monday through Thursday, from 11:30 A.M. to 12:30 P.M. for Aquatic Therapy Groups. For more information call 740-654-2634.
